Intrauterine contraceptives are approved for a variety of durations: 3 and 5 years for IUS; and 2.5, 5, and 10 years for copper IUDs. The approved duration of use is based on clinical trial data, and does not necessarily reflect the true end of life of a device.
Limited data suggest that a LNG-8 provides ongoing contraception up to 4 years, and a LNG-20 provides ongoing contraception for 7 years. Extended use for copper IUDs are more limited. There is limited data showing ongoing contraception of Nova-T up to 5 years, though the failure rate in years 4 and 5 may be higher.
Women whose IUCs are nearing or past the approved duration of use should have their devices replaced on a semi-elective basis. Women can continue to rely upon their IUC while waiting for an appointment for replacement.
Extended use is a possibility for women aged ≥45, but may not be suitable if menstrual management is required.
